You head out of Manly along Sydney Road through Balgowlah and Seaforth, drop down to The Spit, cross the Spit Bridge, then climb Spit Road into Mosman. It's roughly 10–15 minutes in ordinary traffic, and Sydney Road runs almost the whole way, so the route is simple to follow.
Two honest things to plan around: the Spit Bridge is an opening (bascule) bridge that lifts for tall boats several times a day, and Spit and Military Roads can get busy at peak times — so it's worth checking the bridge opening times and leaving a little buffer. When you arrive there's street parking around Spit Road and Brady Street. Prefer not to drive? The 144 bus crosses the Spit Bridge along Spit Road and stops near the clinic; the Manly ferry, by contrast, heads to the city rather than Mosman, so for us the car or the bus is the simplest way.
